dc.description.abstract | spa: Introducción: El crecimiento continuo de las infecciones asociadas a las prácticas en salud es un tema que ha despertado el interés de las organizaciones de control sanitario alrededor del mundo [1,2,3]. En respuesta, el control de riesgos en los procesos de reprocesamiento y esterilización de dispositivos médicos a través de la instauración de protocolos procedimentales ha sido una práctica que ha impactado amplia y positivamente la prevención de brotes infecciosos [8,9,10,11,12,13]. Sin embargo, la implementación de dichas prácticas presupone un reto en términos de tiempo, recursos económicos y recursos humanos [8]. Por esta razón, la automatización de estos procedimientos a través de software y hardware especializados subyace como una alternativa solución a estas limitaciones [14,15,29,30,31,32,33]. Objetivo: Desarrollar una aplicación que permita la identificación de posibles fallos en los procesos de esterilización (a vapor, por óxido de Etileno y por plasma de Peróxido de Hidrógeno) de distintos dispositivos médicos. Metodología: El desarrollo de esta aplicación se realizó definiendo un protocolo acorde con las correctas prácticas de los tres tipos de esterilización mencionados según la OMS, los manuales de buenas prácticas de esterilización para los prestadores de servicios de salud y los postulados de las normativas ANSI/AAMI/ISO 2006. A partir de ellos se realizó un subsecuente planteamiento de posibles errores en las prácticas a manera de algoritmos. Acto seguido, estos se implementaron con una aplicación híbrida escalable en lenguaje Javascript, mediante la metodología de desarrollo de software en espiral (doble iteración), con una arquitectura Front-End haciendo uso de la plataforma de código abierto Expo y del framework React Native; y una arquitectura Back-End haciendo uso del lenguaje PHP, el servicio de base de datos MySQL (tipo relacional) y un servidor de la Universidad del Rosario. Por último, se realizaron pruebas de funcionalidad (manual y de tipo caja blanca), humo, sanidad e integración (método bottom-up); y como prueba de aceptación del usuario, se aplicó la escala de usabilidad SUS en cuatro operarios del área de esterilización a vapor de la Universidad del Rosario luego de interactuar con la herramienta durante la esterilización de seis cargas. Resultados: La aplicación desarrollada presentó una usabilidad de 66,875 para la funcionalidad de esterilización a vapor, lo que la cataloga como marginal. El análisis de promedios por ítem demostró que las fortalezas de la aplicación son la disposición para una mayor frecuencia de uso, la integración exitosa de las funciones, la rapidez de aprendizaje para su uso, la confianza de uso y su baja inconsistencia y dificultad de uso. Mientras que los aspectos a mejorar son la complejidad del sistema y la necesidad de conocimientos previos para su uso. Conclusiones: La implementación de la aplicación por medio de la ingeniería de la usabilidad y el acompañamiento del personal asociado permitió proveer un producto usable, percibido como útil en la identificación de fallos y control de riesgos en la esterilización a vapor, y contribuyó a sentar un precedente para efectuar este tipo de desarrollos en Colombia. | spa |
dc.description.abstract | eng: Introduction: The continued growth of infections associated with healthcare practices is an issue that has piqued the interest of healthcare control organizations around the world [1,2,3]. In response, risk control in medical device reprocessing and sterilization processes through the establishment of procedural protocols has been a practice that has had a broad and positive impact on the prevention of infectious outbreaks [8,9,10,11,12,13]. However, the implementation of such practices presupposes a challenge in terms of time, economic resources and human resources [8]. For this reason, the automatization of these procedures through specialized software and hardware underlies as an alternative solution to these limitations [14,15,29,30,31,32,33]. Objective: Developing an application that allows the identification of possible failures in the sterilization processes (Steam, ETO and Hydrogen Peroxide plasma) of different medical devices. Methodology: The development of this application was carried out by defining a protocol in accordance with proper sterilization practices established by the WHO, the manuals of correct sterilization practices for health service providers in Colombia and the postulates of the ANSI/AAMI/ISO 2006 standards. The subsequent approach to identify errors in the clinical practices was carried out in the form of algorithms. Then, these were implemented inside a scalable hybrid application in Javascript, using the spiral software development methodology with a Front-End architecture built with the open source platform Expo and the React Native framework; and a Back-End architecture using the PHP language, the MySQL database service (relational type) and a server of the Universidad del Rosario. Finally, functionality tests (manual and white box type), smoke tests, sanity tests and integration tests (bottom-up method) were performed; and for user acceptance test, the SUS usability scale was applied to four operators of the steam sterilization area of the Universidad del Rosario after interacting with the tool during the sterilization of six loads. Results: The developed application presented a usability of 66.875 for the steam sterilization functionality, which classifies it as marginal. The analysis of average scores per item showed that the strengths of the application according to users are it is suitable for frequent use, successful integration of its functions, ease of use, confidence within its use, and its low inconsistency and difficulty of use. While the areas for improvement are the complexity of the system and the need for prior knowledge to use it. Conclusions: The implementation of the application using the usability engineering approach and the accompaniment of specialized personnel for its design allowed the investigator to provide a usable product, perceived as useful in the identification of failures and risk control in steam sterilization, and contributed to set a precedent to carry out this kind of developments in Colombia. | eng |
