dc.description.abstractLa extremidad o miembro superior es la parte del cuerpo con mayor movilidad y funcionalidad en las actividades diarias y está compuesto por la cintura escapular (clavícula y escápula), el brazo (húmero), el antebrazo (cubito y radio) la muñeca (carpo),y la mano (metacarpianos), y es considerada como una herramienta mecánica y sensitiva, siendo el principal órgano de manipulación física gracias a sus dos funciones principales de presión y tacto que le permiten realizar movimientos y manipulación de objetos. La pérdida de una extremidad genera en la vida de un individuo cambios devastadores, por las limitaciones que conllevan de efecto traumático, psicológicos asociados con cambios de conducta, falta de seguridad, dependencia reducida, frustración y sociabilidad, entre otros, impidiéndole satisfacer algunas necesidades. Esta situación es aún más marcada en Países de Ingresos Medio Bajos (PIBM), donde oficios manuales como agricultura, ganadería, carpintería, artesanía, entre otras son actividades del entorno laboral promedio, generando un limitante que afectara la condición socioeconómica de la persona con discapacidad. Estudios epidemiológicos recientes revelaron que del 68% al 78% de las amputaciones traumáticas totales, involucran las extremidades superiores, y aproximadamente el 90% de estas son amputaciones parciales de la mano. Según cifras de la Organización Mundial de la Salud (OMS), el 95% de los 40 millones de amputados en los países en desarrollo carecen de acceso a servicios de prótesis apropiados, debido a su condición económica ya que el costo de fabricación de una prótesis, dependiendo del diseño del dispositivo terminal, oscila entre los US $4.000 y US $20.000 (dólares americanos) para los dispositivos propulsados con el cuerpo (conocidos en inglés como Body Powerd). Dicho costo es mucho mayor para los dispositivos mioeléctricas, siendo un gasto inalcanzable para la población de países en vía de desarrollo como lo son la India o cualquiera de las naciones, miembros de la comunidad económica de los estados africanos occidentales (CEDEAO) (15 países) donde del 29% al 70% de los países miembros están en pobreza extrema con ingreso per cápita es de US $2 al día. El objetivo de este trabajo se enfocó en la producción e implementación de una prótesis mecánica y funcional, activada por el movimiento escapular de aducción y abducción del hombro. Entre las características más relevantes se encuentra el bajo costo, lo cual la hace asequible a una población con características económicas limitadas. El diseño, tanto personalizado como novedoso en la implementación de un sistema de tendones internos y dedos flexibles que permiten simular el agarre curvo de la mano. El dispositivo terminal presentado en el contexto de este trabajo permite realizar labores básicas y específicas a las que se ve sometida una persona habitualmente como lo es el agarre de objetos de diferentes volúmenes y pesos. Cabe mencionar, que dicho diseño fue uno de los tres ganadores del concurso internacional de innovación Hand Challenge, proyecto liderado y financiado por la Real Academia de Ingeniería del Reino Unido. EL documento presentado en este trabajo de grado se enfoca en capítulos temáticos, inicia con el estado del arte relacionado con la fisiología de la mano, causas de amputación, las limitaciones del mercado, los dispositivos prostéticos asociados. Continuando con diseños y modelos que inspiraron la creación del dispositivo y las evaluaciones de funcionalidad a las que son sometidas las prótesis de miembro superior. Posteriormente, se encuentra la metodología del trabajo propuesto, la cual enfatiza en el diseño y desarrollo de la prótesis, que a su vez está dividida en etapas (encaje prostético, dedos funcionales, sistema de obtención de fuerza). Para finalizar el capítulo con la selección de los pacientes y las pruebas de validación de la prótesis. Los resultados obtenidos tanto en India (taxonomía de grasp) como en Colombia (taxonomía de grasp y Escala Funcional para Pacientes Específicos), permiten validad la funcionalidad de la prótesis en la realización de tareas cotidianas. Por otro lado, se demuestra que la implementación de un diámetro y longitud apropiada de los dedos permite agarres específicos de menor fuerza en comparación a los diseños comerciales. Se logra concluir que la prótesis presenta una innovación en un agarre funcional de bajo costo que brinda comodidad durante un uso prolongado.spa
dc.description.abstractThe extremity or upper limb is the part of the body with the greatest mobility and functionality in daily activities and is made up of the shoulder girdle (clavicle and scapula), the arm (humerus), the forearm (ulna and radius) the wrist (carpal) , and the hand (metacarpals), and is considered a mechanical and sensitive tool, being the main organ of physical manipulation thanks to its two main functions of pressure and touch that allow it to perform movements and manipulate objects. The loss of a limb generates devastating changes in the life of an individual, due to the limitations that lead to traumatic, psychological effects associated with changes in behavior, lack of security, reduced dependence, frustration and sociability, among others, preventing him from satisfying some needs. This situation is even more marked in Low Middle Income Countries (LMICs), where manual trades such as agriculture, livestock, carpentry, handicrafts, among others, are activities of the average work environment, generating a limitation that affects the socioeconomic status of the person with disabilities. . Recent epidemiological studies revealed that 68% to 78% of total traumatic amputations involve the upper extremities, and approximately 90% of these are partial amputations of the hand. According to figures from the World Health Organization (WHO), 95% of the 40 million amputees in developing countries lack access to appropriate prosthetic services, due to their economic status and because the cost of manufacturing a prosthesis , depending on the design of the terminal device, ranges from US$4,000 to US$20,000 (US dollars) for Body Powered devices. The objective of this work focused on the production and implementation of a mechanical and functional prosthesis, activated by the scapular movement of shoulder adduction and abduction. Among the most relevant characteristics is its low cost, which makes it affordable for a population with limited economic characteristics. The design, both personalized and innovative in the implementation of a system of internal tendons and flexible fingers that simulate the curved grip of the hand. The terminal device presented in the context of this work allows performing basic and specific tasks to which a person is usually subjected, such as grasping objects of different volumes and weights. It is worth mentioning that this design was one of the three winners of the Hand Challenge international innovation competition, a project led and financed by the Royal Academy of Engineering of the United Kingdom. The document presented in this degree work focuses on thematic chapters, it begins with the state of the art related to the physiology of the hand, causes of amputation, market limitations, associated prosthetic devices. Continuing with designs and models that inspired the creation of the device and the functionality evaluations to which upper limb prostheses are subjected. Subsequently, the methodology of the proposed work is found, which emphasizes the design and development of the prosthesis, which in turn is divided into stages (prosthetic socket, functional fingers, force obtaining system). To end the chapter with the selection of patients and validation tests of the prosthesis. The results obtained both in India (grasp taxonomy) and in Colombia (grasp taxonomy and Functional Scale for Specific Patients), allow validation of the functionality of the prosthesis in the performance of daily tasks. On the other hand, it is shown that the implementation of an appropriate diameter and length of the fingers allows specific grips with less force compared to commercial designs.
