Procesamiento de imágenes de mamografía digital para la caracterización de lesiones mamarias

Según la Organización Mundial de la Salud, el cáncer de mama es el cáncer más frecuente en mujeres tanto en países desarrollados como en países en vías de desarrollo. Esta es una enfermedad en la cual las células de la mama crecen y se multiplican sin control. La fundación Susan G. Komen explica que existen gran cantidad de tipos de cáncer de seno, siendo los más comunes el Carcinoma In Situ y el Cancinoma Invasivo, los cuales deben ser estudiados a cabalidad antes de realizar cualquier procedimiento invasivo. Organizaciones e Institutos tal como el Oncohealth Institute de Estados Unidos, explican que el cáncer de mama se puede tratar y curar si se detecta a tiempo donde según la edad se tienen métodos de cribado distintos. El examen clínico y la ecografía mamaria son las opciones principales para todas las edades; para mujeres mayores de 40 años se recomienda realizarse la mamografía la cual, de no ser concluyente con los estudios mencionados anteriormente, necesita de exámenes complementarios tales como la biopsia y/o la tomografía. En estos estudios se utiliza un sistema estándar para describir los resultados y hallazgos llamado ¨Breast Imaging Reporting and Data System¨ (BIRADS), el cual clasifica los resultados en categorías numeradas de 0 a 6.
En este trabajo se propone un método para la caracterización de las lesiones mamarias siguiendo el estándar BIRADS, a partir del procesamiento y análisis de imágenes de mamografía digital. Se utiliza la base de datos del ¨Centro de Imagenología Avanzado Mérida¨ de Mérida, Venezuela, reagrupando 300 imágenes en 3 categorías: BIRADS 1, BIRADS 2-3 y BIRADS 4-5. Las imágenes correspondientes a las últimas dos categorías fueron segmentadas manualmente por un experto, con el objetivo de tener imágenes de referencia para la evaluación del desempeño del algoritmo.
En la etapa de preprocesamiento del método propuesto, se realiza un recorte y una mejora de contraste de las imágenes, a través del uso de filtros adaptativos. Posterior a esto, se realiza la segmentación del músculo pectoral mediante la utilización de segmentación por crecimiento de regiones y operadores morfológicos. Seguido, se segmentan lesiones tanto a nivel del músculo como a nivel mamario utilizando la Transformada de Wavelet Discreta, para detectar la presencia de microcalcificaciones. Asimismo, se utiliza una combinación de la técnica de crecimiento de regiones y una umbralización multiumbral para segmentar lesiones densas y otros tipos de lesiones. Finalmente, se extraen características de textura estudiando la distribución de intensidades a partir del histograma de las masas encontradas y características morfológicas, indicando la variación de la forma de la estructura exterior, en las que se destacan el perímetro, eje mayor, eje menor, centroide y área en milímetros.
Para evaluar el desempeño del método propuesto, se comparan las imágenes segmentadas manualmente y las segmentadas automáticamente, obteniendo un índice de similitud de Sørensen-Dice de 0.73. Se considera este un resultado favorable, tomando en cuenta que solo es posible realizar una delimitación aproximada del área de la lesión en las imágenes de mamografía, ya sea de forma manual o automática. According to the World Health Organization, breast cancer is the most common cancer in women in both developed and developing countries. Clinical examination and breast ultrasound are the main options for all ages; For women over 40 years of age, a mammogram is recommended, which, if it is not conclusive with the studies mentioned above, requires additional tests such as biopsy and/or tomography. In these studies, a standard system is used to describe the results and findings called ¨Breast Imaging Reporting and Data System¨ (BIRADS), which classifies the results in numbered categories from 0 to 6. In this work, a method for the characterization of of breast lesions following the BIRADS standard, from the processing and analysis of digital mammography images. The database of the "Centro de Imagenología Avanzado Mérida" of Mérida, Venezuela, is used, regrouping 300 images in 3 categories: BIRADS 1, BIRADS 2-3 and BIRADS 4-5. The images corresponding to the last two categories were manually segmented by an expert, with the aim of having reference images for the evaluation of the algorithm's performance. In the preprocessing stage of the proposed method, a cropping and contrast enhancement of the images is performed, through the use of adaptive filters. After this, the segmentation of the pectoral muscle is performed using region growth segmentation and morphological operators. Next, lesions are segmented both at the muscle level and at the breast level using the Discrete Wavelet Transform, to detect the presence of microcalcifications. Also, a combination of region growth technique and multithreshold thresholding is used to segment dense lesions and other types of lesions. Finally, texture characteristics are extracted by studying the distribution of intensities from the histogram of the masses found and morphological characteristics, indicating the variation in the shape of the outer structure, in which the perimeter, major axis, minor axis, centroid stand out. and area in millimeters. To evaluate the performance of the proposed method, manually segmented and automatically segmented images are compared, obtaining a Sørensen-Dice similarity index of 0.73. This is considered a favorable result, taking into account that it is only possible to make an approximate delimitation of the area of the lesion in the mammography images, either manually or automatically.
